Send us quick email, including the following information.  We will get a hand-written card in the mail, acknowledging your donation to LOMCP.

• Is this an honor or memory donation?

• Name of the person this donation is in honor/memory

• Name and address of the person(s) to receive the acknowledgement card

• Relationship of the acknowledgement card recipient to the person of honor/memory (mom, son etc…)

• Your name or group making the donation
